  • Click on Blank Application Form Downloadfrom the list. ( issue detail ) .

  • Choose ‘ IND-INDIVIDUAL ‘ from the Investor Status drop down.

  • Enter the Captcha Code.

  • Before tick mark Read and agree to the terms and conditions.

  • Click on Download Button

  • Finally, Check on your Download Folder.
